Permission Form for Impromptu Off-Site Activities

I am asking parents to give a 'blanket permission' for their children to go on numerous outings to the Millisle Beach Park during Term 3 so that teachers can take their pupils out as and when a good opportunity arises. A detailed risk assessment for these trips can be viewed online; please read the document before completing the permission form.
