Parent/Guardian Consent and Pupil Pledge

(1) Parent/Guardian Consent: To enable us to comply with our obligations under the General Data Protection Regulation, we are required to obtain express consent to the use of a pupil’s image for example in school displays, performances, newsletters, prospectus and our social media platforms. (2) Pupil Pledge: On behalf of children aged under 13, Parents/Guardians should confirm their agreement with and willingness to adhere to the statements listed below by completing and submitting this form. Before completing, please read the school policy for the use of photographs, video, internet and ipads.

I confirm that I have read and understand the school policy "Consent & Pupil Pledge Booklet-Photograph, Video, Internet and iPad Usage" in relation to use of images of my child and the use of Internet and ipads within school by my child.

I give permission for photographs of my child to be used in the printed school newsletter and I understand that it is posted in PDF format on the School website and emailed to parents on request.

I give permission for photographs, voice recordings or videos of my child to be used on the School's website, virtual learning environment and our schoolmanaged social media portals, for example: as part of a school trip blog.

My child may be named in the caption or article associated with the image in the school newsletter.

I give permission for photographs, voice recordings or videos of my child to be used on the School's website, virtual learning environment and our school-managed social media portals, for example: as part of a school trip blog.

My child may be named in the associated captions or articles on the website, virtual learning environment or school-managed social media portals.

My child’s image, voice or work may be used in school promotional materials, for example: prospectus, other publications that we produce for promotional purposes.

I give permission for visiting media organisations to take photographs or video footage of my child and use them in local or national publications, on websites and on radio or television programmes

My child may feature in footage recorded for the purposes of teacher training which is shared with other teachers in the school or externally to help raise teaching standards.

I understand that it is my responsibility to retain any photos/videos recorded by myself for family use only. The content should not be posted onto social media sites, such as Facebook.

I understand that I must withdraw my child from any ‘public’ event if I do not wish my child to be included in photographs/ videos made by parents or others attending the event.

I understand that I may withdraw my consent without explanation.

I understand that the school will implement my decision throughout my child’s time in Millisle Primary School unless I inform the school otherwise, in writing.

On behalf of my child, I agree that he/she will adhere to the Rules for Responsible Internet and iPad Usage as defined in the "Consent & Pupil Pledge Booklet-Photograph, Video, Internet and iPad Usage" booklet.
